Iris Expands Multi-Lingual Advance Care Planning Program with Brand New Day Health Plan

Brand New Day has expanded its partnership with Iris to deliver industry-leading Advance Care Planning services to the plan’s Medicare members across its extensive provider network of over 7,300 PCPs and 22,300 specialists across 12 counties in California.

Since the initial launch in May 2019, Iris has teamed with the plan’s Care Managers and Health Coaches to offer advance care planning to its Medicare Advantage members who suffer from a life-limiting serious illness. “The partnership with Brand New Day has been highly successful and we are delighted to now expand across their entire member base” says Elaine Cannizzaro, VP of Member Operations at Iris. “In working alongside the plan’s care management organization, we have flourished in connecting with members and delivering these important services.”

One of the features of this partnership is the diverse population served. A significant portion of the plan’s population speaks a native language other than English. Iris serves this population with facilitators that are bi-lingual in Spanish and English, and uses interpreters to support a wide variety of other languages. By having support for multiple languages, Iris provides Advance Care Planning attuned with cultural sensitivity.
