The session will focus on the spending for serious illness by health plans that is tied to unwanted, unnecessary, and non-beneficial care. While palliative care has proven to improve quality and lower costs in the face of serious illness, it remains a scarce resource and often delivered inefficiently. This session will dispel common misconceptions about advance care planning and illustrate through real-world case studies how it can be delivered with scale and consistency to vulnerable populations
