AUSTIN — November 2019 – Iris, Inc. (www.irisplans.com), a visionary leader in Advanced Care Planning (ACP) solutions for health plans and other risk-bearing organizations, has partnered with IlliniCare to provide ACP services for IlliniCare Health’s Dual Eligible Special Needs (DSNP) members.
IlliniCare chose Iris because of its focus on providing an excellent member experience, as well as Iris’ results in reducing unnecessary hospitalizations and ED visits, and proof that members who have gone through Iris’ advance care planning program receive less intensive care in alignment with their wishes. Key in IlliniCare Health selecting Iris was its consistent program results with gaining completed and fully executed ACP documentation, an area that IlliniCare has prioritized.
Consistent with its practice of tailoring solutions to the unique needs of each population, Iris developed a customized outreach and engagement program that integrated the IlliniCare member’s case manager to improve results.
Additionally, Iris developed and implemented a unique voice signature process which improved completion of ACP documents for this unique population. Iris has proven processes for making sure that all signed documentation is distributed to the point of care for each member.
